Voluntary Life and AD&D
Should you wish to have additional life insurance coverage above the employer-paid life policy you have the opportunity to purchase voluntary life and AD&D insurance for yourself, your spouse, and/or your dependent children. Your cost for this coverage is based on the amount you elect and your age. You must purchase voluntary life and AD&D insurance for yourself in order to purchase spouse and/or dependent child(ren) coverage. If you did not enroll in this coverage when you were first eligible, you will be subject to medical underwriting.
Evidence of Insurability
Evidence of Insurability (EOI) is the application process in which you provide information on the condition of your health or your dependents’ health in order to be approved for coverage for voluntary life and AD&D coverage.
